
Positive Pathways Program
The COVID-19 Positive Pathways Program is a Victorian state-wide program for assessment and management of
COVID-19 patients within the community.
The program is designed to provide community-based care for low risk COVID-19 patients, with escalation methods for further health care if required.
Kilmore District Health (KDH), in conjunction with Goulburn Valley Public Health Unit has developed a local COVID-19 Positive Pathways Program to provide support and monitoring to COVID-19 positive patients at home.
The program has a number of nursing and administration staff and includes:

providing an at home Pulse Oximeter machine to monitor oxygen levels and a thermometer

daily screening of patients to identify any changes to symptoms

ensuring support is in place to access healthcare if required

identifying any signs of deterioration and escalation of care if needed, and

helping patients to access and manage any other health care and/or social needs during their isolation period.
KDH has also linked with local pharmacies to ensure the medication requirements of patients can be maintained whilst they are recovering at home, in isolation. It is important to ensure patients are receiving their medications they may need in relation to their COVID-19 positive diagnosis as well as maintaining regular ongoing medication for pre-existing medical conditions.
